EAGLE VISA CARD

Eagle Advancement Institute’s pre-paid Visa card (valid only for use at Eagle Advancement Institute centers). This will allow friends, family, community groups, and employers to contribute to the cost of your procedure with the assurance that the funds will be used only for your recovery. The Eagle Advancement Institute Visa card is only good for use at our facilities. It may be loaded from multiple sources, wire transfer, ACH, credit card, and from multiple people in different geographic locations. CHECKS

Eagle will also accept cashiers, certified, and personal checks at the center. Personal checks cannot be considered the “same as cash” and if the funds are not available upon your arrival at the center your deposit may not be refundable.

DEPOSITS/CANCELLATIONS

All cancellations must be received in writing 3 business days prior to intake day. We will accept a procedure date change at that time. The new procedure date must be confirmed within 30 days from original procedure day.

EAGLE FLIGHT FOR FREEDOM

Eagle will offset your air fare for continental US travel .25 per mile when utilizing Clarity Advantage. Calculation is made utilizing patient’s residence address to Detroit Metropolitan Airport. Offset will be paid ONLY after procedure is completed (at discharge) and paid in full.
